Should you utilize your insurance, please be aware of the following:

• You will need to provide your insurance card and identification.

• You will need to be aware that your insurance company requires the following information: when you attend session, your mental health diagnosis, your symptoms, behaviors, impairments, and progress that you are making.

• You will need to be responsible for any co-pays per your insurance plan.

• You will ultimately be responsible for checking your mental health benefits with your insurance company.

Sliding Scale

If you are in need of a sliding scale, please do not hesitate to ask during your consultation. 10% of our spots are reserved for clients needing a sliding scale for fees.
